Business Permit Application
- Barangay Clearance on Business (Location of Business)
- Community Tax Certificate (Cedula): This can be obtained from the Barangay or Municipal Treasurer’s Office.
- Proof of Business Registration (DTI/SEC/CDA)
- For Sole Proprietorships: Department of Trade and Industry (DTI) Business Name Registration.
- For Partnerships and Corporations: Securities and Exchange Commission (SEC) Registration.
- For Cooperatives: Cooperative Development Authority (CDA) Registration.
- Income Tax Return (ITR) – BIR
- Financial Statements of Sales (FS) – Duly signed by Authorized Person
- Real Property Tax/Tax Dec or Tax Clearance – MTO & Assessor’s Office
- Photocopy of the previous Business Permit or Official Receipts
- CoC (Certificate of Compliance) – For Fuel/Gas Stations
- NTC VAS Certificate of Registration – For Internet Service Provider (ISP)
- For Business Retirement/Closure – Brgy. Certification for Business Retirement (No Unpaid Taxes)
